Last time, I reverted a save, and MUCH later, the issue happened again.

But this time, I think I’ve noticed a common thread between these two instances of the bug cropping up.

The last time it happened, one of the things I did involved doing a survey quest on a random planet with life, with an active companion (one of the Constellation companions, in this case).

The second time, I was on a genetic sampling quest (not spoiling how you get that classification of quest) on another such planet with another Constellation companion.

I don’t know how much of this common thread could be causing this bug, but for those who’ve had to revert saves over it, I hope this revelation helps somehow, wether through general caution or further testing.

Thanks for your work! Mind DM’ing me the name of the quest? My current playthrough involves a lot of ship fights and boarding enemy ships, so I’d hate to fall victim to the bug and may skip the quest altogether.

Oh, I Just was saying that surveying planets in general while you have an active companion can risk triggering the bug (maybe it even is triggered by actual combat with miscellaneous alien critters, can’t confirm that, though).

The quests related to surveying planets or critters are fine, as long as you don’t bring a companion.

However, if you want to try the quests I’ve used for this testing, both are radiant/miscellaneous quests you can get after completion of the Vanguard questline.

I tested boarding using a quest you can get by talking to Commander Tuala in New Atlantis.

The objective that gives you the gene-sampling quest involves talking to a guy named “Perceval” (you’ll know him when you go through that questline)
